Abstract. A previously well 84 year old man without prior history of abdominal trauma, instrumentation or surgery presented with localized biliary peritonitis (biloma). An associated bile duct stone was detected by abdominal ultrasound and endoscopic retrograde cholangiopancreatography and was successfully treated with systemic antibiotics and moved, and the percutaneous biloma drainage catheter was left in place. After 3 weeks of drainage, ERCP was reat-tempted and drainage of the bile duct was achieved. A large leak from a main left branch duct was found. The leaking duct was treated with a plastic stent that extended into the biloma cavity. After 2 days, the patient was dis- Biloma is defined as any collection of bile outside the biliary tree, usually resulting from surgery complications and abdominal trauma. Spontaneous occurrence of bilomas is rare, occasionally associated with choledocolithiasis. The present report describes a case of spontaneous biloma, whose diagnosis was radiologically confirmed.
